Ketoconazole is an antifungal shampoo that has demonstrated significant hair loss benefits beyond its antifungal action. A landmark 1998 study found ketoconazole 2% shampoo increased hair shaft diameter comparably to 2% minoxidil. It reduces scalp inflammation and has mild anti-androgenic properties, making it a popular adjunct in most serious protocols.
Ketoconazole inhibits ergosterol synthesis in fungi (treating seborrheic dermatitis and dandruff), which reduces scalp inflammation. Separately, it has been shown to block androgen receptor activity in the scalp, mildly reducing the local DHT effect on follicles. Reduced inflammation also improves the environment for follicle growth.
