Scalp microneedling has strong clinical evidence for hair regrowth, with multiple studies showing it outperforms minoxidil alone when combined. The mechanism involves platelet-rich plasma (PRP) release, Wnt/β-catenin pathway activation, and micro-injury-induced growth factor release. However, it carries the most critical safety timing requirement of any hair loss treatment: minoxidil MUST NOT be applied for 12–24 hours after needling.
Microneedling creates controlled micro-injuries in the scalp dermis. This triggers the body's wound-healing response — releasing growth factors including VEGF, PDGF, and IGF-1 that activate hair follicle stem cells. It also increases scalp blood flow, stimulates collagen remodeling, and dramatically increases topical penetration (including minoxidil). The increased permeability is both its benefit and its main risk.
